Release from liability

I, the undersigned, do hereby willingly take part/allow my child to take part in the present karate event organized and supervised by the Kyokushin Philly Fight Club, KUSA, Justin Hagen and XL Sports. I recognize that potential risk of injury is inherent to the practice of any martial art and I fully and unquestionably take on full responsibility for any damage, injury, or other offenses that I/or my child may sustain or incur during this event. I renounce any legal action or claim against the instructors,employees, agents, members and authorized guests of the Kyokushin Philly Fight Club, KUSA, Justin Hagen, BucksMont TKD and XL Sports.

I fully understand that this is a FULL CONTACT event both in ALL divisions including youth.

Lastly, I understand and I accept that, in the event of an injury, the medical assistance I/my child would receive on site would be limited to first aid care.

I have fully read and understood the content of the document above and I willingly agree to sign this release from liability in order to participate/allow my child to participate in this karate event organized by the Kyokushin Philly Fight Club, KUSA, Justin Hagen, BucksMont TKD and XL Sports.

I hereby give permission for images of myself and/or my child, captured during this event through video, photo and digital camera, to be used for the purposes of the Kyokushin Philly Fight Club, KUSA, and XL Sports promotional material and publications as well as social media, and waive any rights of compensation or ownership thereto.
