Release from liability

I, the undersigned, do hereby willingly take part in the present karate event organized and supervised by Kyokushin Philly's Kyokushin Fight Club, KUSA, Justin Hagen and the USKA. I recognize that potential risk of injury is inherent to the practice of any martial art and I fully and unquestionably take on full responsibility for any damage, injury, or other offenses that I may sustain or incur during this tournament. I renounce any legal action or claim against the instructors,employees, agents, members and authorized guests of Kyokushin Philly, KUSA, Justin Hagen, BucksMont TKD and XL Sports.

 

I fully understand that this is a FULL CONTACT event both in the Semi Knockdown and Full Knockdown divisions.

 

Lastly, I understand and I accept that, in the event of an injury, the medical assistance I would receive on site would be limited to first aid care.

 

I have fully read and understood the content of the document above and I willingly agree to sign this release from liability in order to participate in this karate tournament organized by Kyokushin Philly, KUSA, Justin Hagen, BucksMont TKD and XL Sports.

 

I hereby give permission for images of myself and/or my child, captured during this event through video, photo and digital camera, to be used for the purposes of Kyokushin Philly, KUSA, and the USKA promotional material and publications as well as social media, and waive any rights of compensation or ownership thereto.
